Payments
Payment for services are processed through Swipe Simple. You will be sent an invoice the following Friday after your appointment. Please keep payment confirmation for your records. You will be reminded for late or missed payments.
Canceled appointments with at least 48 hours of notice can be rescheduled when there is an available appointment.

Before your first Visit
To establish care with us, you will need to complete a brief pre-screen form to send back to us. Additionally, you will have to check your insurance if we are one of the preferred providers. Upon receiving this request, a member of our team will contact you. Prior to your appointment, you will complete a detailed intake form so you can get a comprehensive medical and psychiatric history. It is required that the new client paperwork be completed and submitted at least five days prior to your appointment. Initial appointments for new clients can be anxiety producing. However, upon arrival you will be greeted with a calming and relaxed atmosphere. New client evaluations are priced per your evaluation. New client evaluations are typically 60 minutes. No medications is prescribed until evaluation has been completed. Please note we do not participate in legal issues, court cases, or domestic disputes. Our primary role is to diagnose and treat medical conditions.
Cancellation & Rescheduling Policy
We require a 48-hour notice for cancellations of appointments so we can accommodate other clients. If you no-show or cancel your appointment with less than 48 hours notice, you will be charged $110.
No refill of medication will be done with no-show and cancelled appointments. Please call the office to reschedule your appointment upon collection of payment.
Medication Refills
You are expected to see your clinician as medically necessary, and until a therapeutic level of medication is achieved. Medications are refilled during office appointments only. Patients are given enough medication to last until next appointment. We do not give a one-year refill to avoid adverse reactions.
Additionally, all specialty medications need to be monitored. If you don’t show up or cancel your appointment but you need a refill, you will be billed for the appointment.
We do not prescribe long term use of benzodiazepines, Schedule III drugs, or opioid medication. Our goal is to make you healthy and taper off such medications to avoid addiction.
Patient Rights
Clients are treated with the highest quality of psychiatric care available. We will provide you with a thorough review of your current complaints. By the end of your visit, we will discuss impression and treatment options, including counseling referrals. No medication is given until the evaluation is completed.
One of the most important aspects of a therapeutic relationship is to make sure we have a good and fit provider-client relationship, so the initial visit is vital in determining this fitness. To have a therapeutic relationship, we see new clients face-to-face. These visits will be conducted as medically necessary for all clients alternating between face-to-face and telepsychiatric videoconferencing.
Amodeus Healthcare takes adequate protection to safeguard the client’s information seriously. We are bound by law to protect our clients’ confidentiality. Any disclosure of your treatment to others will require written consent from you. Furthermore, the law mandates and legally binds us to report to appropriate authorities any indications of child abuse, elder abuse, or form of threat.
Urine drug screens are performed at random by your provider. This most often occurs at your appointment and is needed to ensure compliance with the treatment plan. This also allows trust to be established between you and your provider.
